I authorize my previous employers to release and forward the information requested of these documents concerning my:
1. Alcohol and Controlled Substance Testing,
2. Safety Performance History Records Request,
and
3. Accident History records within the previous 3 years from today.,
4. FMCSA Pre-Employment Screening Program,
I authorize Tuf Transport Service, Inc. to access the PSP system to seek information regarding my commercial driving safety record and information regarding my safety inspection history. I understand that I am consenting to the release of safety performance information including crash data from the previous (5) years and inspection history from the previous (3) years. I understand and acknowledge that this release of information may assist Tuf Transport Service, Inc to make a determination regarding my suitability as an employee.
